My Role
I am currently a UX Designer on the Care Transitions team which manages the experiences of two products:
nH Discharge streamlines care transitions by matching patients with post-acute providers and creating/sending level of care referral packets electronically.
nH Intake makes managing patient referrals and measuring performance a much simpler & quicker process by securely connecting to nH Discharge online.
Key tasks:
Create intuitive, user experiences with strong, conceptual design thinking
Collaborate with front-end UI and back-end developers, product management, and other internal teams to meet highly technical business objectives
Translate ideas and concepts into workable wireframes, user task flow diagrams or journey maps, fully interactive prototypes, and functional design specifications
Communicate and present user needs and optimal design solutions to key stakeholders
